Home Stretch

Days: Monday—Friday
Times: 8am—4pm
Ages: 17.5—21

BYFH is pleased to be offering our community the Home Stretch WA program. Home Stretch gives young people who are leaving out of home care the choice to access continued support through a dedicated transition coach into adulthood.

This program provides one-to-one support through a personalised case plan focused on post care supports as young people move towards independence. Home Stretch participants have access to Invest in Me funding, Staying on Subsidy, or Housing Allowance (where eligible), support and advocacy, and proactive work on developing their support circles through kin, community, culture and services.

Home Stretch is a WA initiative that’s been co-designed with young people with lived experience with a particular focus on young Aboriginal people and their families. The Department of Communities has partnered with community service organisations and Aboriginal Community Controlled Organisations to deliver the Home Stretch WA program across the State.
